Medical abortion at home

In Norway, women have a statutory right to abortion on request up to the end of the 18th week of pregnancy. Later in the pregnancy, the application must be considered by an abortion board.

It can be difficult to decide whether to continue or terminate a pregnancy. If you need someone to talk to, you can call Amathea on 90 65 90 60. Amathea is a free counselling service for people making decisions related to pregnancy and abortion.

For a medical abortion to take place at home, you must be over 18 and in good health. You must be able to understand and speak Norwegian or English, and be able to have an adult with you. Home abortion is an option up to week 9 of pregnancy. At some hospitals, home abortion is possible up to week 10.

Referral and assessment

Planned follow-up for an unintended pregnancy usually starts with your GP, who refers you to the hospital. You can also contact the hospital directly for an examination and further follow-up; a referral from your GP is not necessary.

It is important that you contact your GP or the hospital directly as soon as possible if you have an unintended pregnancy and are considering terminating it.

At your GP’s office

Your GP will assess the length of the pregnancy before referring you to the hospital.

At the hospital

At the hospital, you will have a consultation with a nurse and/or gynaecologist. A gynaecological examination with an ultrasound will be performed to determine the length of the pregnancy, and you will also be tested for chlamydia.

You will receive thorough information and have a consultation before the final treatment method is decided. Usually, the abortion is induced with medication. You will be asked to sign the declaration form “Request for termination of pregnancy”.

Before

Initial treatment

You have been examined and assessed at the hospital, and it has been agreed that the abortion will take place at home using medication. Two different medications are used.

  • Mifegyne, which starts the abortion process
  • Cytotec, which helps the body complete the abortion

The first part of the treatment takes place at the hospital. You will be given a Mifegyne tablet to swallow; once you have taken it, it is too late to change your mind. Before you leave the hospital, you will be given Cytotec and painkillers for the rest of the treatment at home.

You may experience bleeding and menstrual-like abdominal pain over the next few days. Use sanitary pads, not tampons, when bleeding. If you need pain relief, you can take 1 gram of paracetamol up to four times a day. In a few cases, the abortion occurs without any further treatment being needed.

Contact the hospital if:

  • You vomit within 2 hours of taking Mifegyne
  • The abortion occurs before further treatment has been started

During

Further treatment at home

Two days after the abortion process was started at the hospital, you continue the treatment at home. It is essential that you have an adult with you.

What should you do?

  • Start the treatment in the morning
  • insert 4 Cytotec tablets high into the vagina
  • Wait 10–15 minutes before standing up; after that, you should stay upright to speed up the abortion process
  • You can eat, drink and otherwise go about your normal activities

The abortion usually occurs 4–6 hours after the Cytotec tablets have been inserted. If you have not started bleeding after more than 6 hours, call the hospital for information and to make further arrangements.

NB! You may experience severe menstrual cramps, and heavy bleeding is normal. If you feel unsafe, contact the hospital.

Please note: You must not drive that day.

After

You can go about your normal activities if you feel well enough.

Your period will usually return after 4–6 weeks. It is not unusual for the first period to be heavier and last longer than you are used to.

Hormonal contraception can be started as soon as the abortion is complete. An IUD can be inserted after 4 weeks, usually by your own doctor.

Follow-up

You should take a pregnancy test 4 weeks after the abortion to confirm that the pregnancy has ended.
