This is a healthcare service where you complete digital questionnaires about your health online. You will only be called in for in-person appointments at the hospital when necessary for your follow-up and treatment.
To assess your health and what you need, we will send you a link to a questionnaire by SMS. This is how it works:

You do not have to pay a co-payment when you use the questionnaire. You must pay a co-payment when an in-person appointment with us is necessary.The same applies to telephone consultations.
If you use a medical aid, in some cases we will read data from the device.
If everything looks fine, there is no need for an in-person follow-up appointment at the hospital.
