Sørlandet Hospital uses forms during assessment and treatment. These may be completed before your first appointment with us, during assessment and treatment, and sometimes after treatment has ended.
 
This also applies if you are a parent, another relative or a teacher of children and young people undergoing assessment or treatment.
 
You complete the forms digitally through secure login via ID-porten in the Checkware system.
 
Please note that your responses here will not result in an immediate response from Sørlandet Hospital. If you need immediate help, contact the out-of-hours medical service or call 113.

Sørlandet Hospital offers digital follow-up for patients by sending digital questionnaires. You enter your responses on a secure website, and the information is transferred directly to your patient record.

You will receive an SMS from Sørlandet Hospital asking you to visit sshf.no and click “Digital self-registration for patients”. You can use a mobile phone, tablet or PC.

If you do not have BankID, contact the department that sent you the form.

If you have received a message asking you to complete a form on behalf of a child or someone else who cannot respond themselves, log in with your own BankID​.

  • After logging in, you will be taken directly to the questions you need to answer.
  • Your answers are saved automatically.
  • You can continue filling out the form later if necessary.
  • If you need to complete several forms, the next form will open automatically.
  • You cannot change your answers after submitting them. If you still wish to change your answers, contact your healthcare provider.
