Sørlandet Hospital uses forms in investigations and treatment processes. This can be before your first appointment with us, during the investigation and treatment, and sometimes after treatment has ended.
 
This also applies to you if you are a parent, other close relative, or teacher of children and young people who are undergoing investigation or treatment.
 
You fill it out digitally, through secure login via the ID-port portal in the Checkware system.
 
Please note that your responses here will not result in an immediate response from Sørlandet Hospital. If you need immediate help, contact the emergency medical service or 113.

Sørlandet Hospital offers digital follow-up to patients by sending digital questionnaires. You register your answers on a secure website, and the information is transferred directly to your patient record.

You will receive an SMS from Sørlandet Hospital. It will ask you to go to sshf.no and click on "Digital self-registration for patients." You can use a mobile phone, tablet, or PC.

If you do not have BankID, contact the department that sent you the form.

If you have received a message to answer a form on behalf of a child, or others who are unable to answer themselves, you should log in with your own BankID.

  • You will directly access the questions to be answered after logging in.
  • The answers are saved automatically.
  • You can continue filling it out later if necessary.
  • If you have to answer more forms, the next form will be opened automatically.
  • You cannot change your response once it has been submitted. If you still wish to change your response, you must contact your healthcare provider.

The answers are read by healthcare professionals who are investigating or treating you. Sensitive information is subject to the duty of confidentiality.
